Length of Operation:
Year your organization as it exists today was created

Staff:
The number of people who work at your organization (if you wish, you may specify between full and part-time, volunteer and paid, etc.)

Area Served:
List each County and Independent City in your primary service area (please call if impractical)

Description:
Describe your organization and its activities as you see fit, but make an effort to include the following information, if appropriate:

  • Your organization's mission
  • Primary population served
  • Services provided
  • Important relationships with other organizations ( if any)
  • Accomplishments and current activities

If possible, keep entry to no longer than 250 words.
